  • Preventative Care
  • Sealants
  • Fillings
  • Routine Extraction (Single Tooth)
  • Local Anesthesia
• Orthodontics:
  • Pre Orthodontia Treatment
    • $150 copay
  • Comprehensive Orthodontia Treatment
    • $2000 copay

• You don’t want to see a dentist outside of Kaiser Dental offices.

With Kaiser Dental, you’ll need to visit one of these Kaiser Dental locations:

• Annual Maximum:
  • $1,500
• Deductible:
  • $0
• General Office Visit:
  • $15 per visit
• These services are covered with the office visit copay:
  • Routine Exams
  • X-rays
  • Preventative Care
  • Fillings
• These services are covered with 20% coinsurance:
  
  • Crowns
  • Routine Extraction (Single Tooth)
  • Dentures
• Orthodontics:
  • 50% of charges up to the $1,500 annual maximum - member is responsible for all of the remainder

  • 50% coinsurance up to the Lifetime Maximum of $1,500
