Unum’s Life Insurance provides financial support to your beneficiaries in the event of your passing, offering coverage that helps secure their financial future.
L&C offers coverage for each benefit-eligible employee at 1.5x their annual salary up to a maximum amount of $250,000.
This coverage is fully paid by Lewis & Clark, with no employee contribution necessary.
AD&D insurance from Unum provides financial support in the event of accidental death or severe injury, including loss of limbs, sight, hearing, or speech, offering coverage for both you and your loved ones.

Monthly Benefit: 60% of your monthly earnings, up to $10,000 per month.
Elimination Period: Benefits begin after 90 days of continuous disability.
Maximum Benefit Duration:
Benefits are paid until Social Security Normal Retirement Age if disability occurs before age 62.
If disability occurs at age 62 or later, benefits are paid for a set number of months based on age.
Additional Features
Rehabilitation & Return to Work Assistance: Extra support and benefits to help transition back to work.
Dependent Care Expense Benefit: Up to $350 per dependent per month (max $1,000/month) for childcare while participating in the return-to-work program.

Pre-Existing Condition Limitation: Disabilities caused by conditions treated in the 3 months before coverage starts may not be covered if disability occurs within the first 12 months of coverage.

Evidence of Insurability (EOI) Requirement for Late Enrollment
If you do not enroll in Optional Life Insurance during your initial benefits election, you must complete an Evidence of Insurability (EOI) form.
Also known as a Unum Statement of Health, this form is required for any late enrollment.

After signing up for the plan, the form will be available in Workday.

You will see your life insurance plans listed. Click on the one you’d like to edit, then click “Confirm and Continue” at the bottom of the page.
You will see two sections, one for primary and one for secondary beneficiaries. You are able to add one or multiple individuals in each section and assign them each a percentage of the benefit. All percentages must add up to 100 in each section.
Check the I Agree box to provide an electronic signature confirming your changes.
Click Submit.
Click Done to complete the task or Print to launch a printable version of the summary for your records.
