Welcome Letter – Undergraduate
On behalf of the Student Health Center, Student Counseling Center, and Health Promotion and Wellness, welcome to Lewis & Clark College!
We are here to promote your health and well-being, along with the health of our community. Please remember to complete the following tasks related to that goal:
You will receive email notification when you are able to register for the portal. Before registering, you must set up your Lewis & Clark email account and then wait at least 24 hours. To set up your Health Information Portal account, please check out this help sheet for registration instructions and troubleshooting tips.
The portal is a secure platform that will allow you to enter immunization information, make appointments at the Student Health Center, and access lab results and visit summaries from Student Health Center visits. You can also use the portal to complete forms or to message your providers for both the Student Health and Student Counseling Centers.
Measles or MMR (Measles, Mumps, Rubella) immunization requirement for ALL students attending one or more classes in person (if born after 1956):

Students must enter dates of TWO (2) doses of MMR or measles vaccine.
- The first dose must have been administered on or after 12 months of age, AND
- The second dose must have been administered a minimum of 28 days after the first.
- If the timing of your MMR vaccines does not meet the above criteria, a measles lab (titer) report indicating immunity satisfies the measles requirement
Students who do not meet the Oregon law requirements by stated deadlines for their program will experience registration holds or be restricted from attending classes.
Before arriving on campus, please enter your two measles/MMR immunization dates in the Health Information Portal. You will receive email notification when you are able to register for the portal:

Click on Immunizations in the left column (syringe icon) to enter your dates or to request an electronic inquiry from up to three state immunization registries.
- If you received immunizations in Oregon, they will be automatically entered if available. Please check the portal to confirm whether your immunization information includes the measles (MMR) vaccination.
- You can request records electronically (not available from: Alaska, Arkansas, the District of Columbia, Hawaii, Maine, New Hampshire, or Vermont). To use this service, you must enter your cell phone, email, and birthdate along with up to 3 states where you received vaccinations.
- If you don’t have dates and cannot use the electronic inquiry, contact your medical provider, most recent school, or google “ state name immunization registry” to request this information.
- You can enter other vaccination/immunization data, but they are not required.

- You will receive routine email reminders until you have met this requirement. If you think you have met the requirement and continue to get reminders, please contact health@lclark.edu.
Though not required, for the health and safety of you and our community, if you have lived in or traveled to a country or territory with a high incidence of tuberculosis disease in the past 5 years we strongly encourage you to confirm non-active TB status with a Quantiferon/ Interferon-Gamma Release Assay (GFT/IGRA) blood test or TB skin test (CDC TB Risk Information).
Oregon state law requires us to provide ALL students with information on vaccine-preventable diseases known to occur in individuals between 16 and 21 years of age. We encourage you to review the college immunization checklist from the Oregon Health Authority, and then talk with your health care provider about what additional vaccines might be appropriate. See also the American College Health Association (ACHA) recommendations for immunizations for college students.
The Student Health Center is a convenient and low-cost option for primary care or occasional acute care for illness or injury. At any time before you arrive on campus, or before your first visit to the Student Health Center, complete your Health History Form.
- Go to the Health Information Portal, and click on “Forms” on the left panel (pencil icon). Complete the Health History Form under section 1: Required Health Forms for New LC Students
Once you have settled on your insurance plan for the upcoming school year, and anytime you add or change your insurance plan, please return to the Health Information Portal to share your insurance information with us (sharing this information is recommended, but not required to receive on campus health or counseling services).
- Go to “Insurance” on the left panel (umbrella icon).
- Have your insurance card and policy holder (subscriber) information (this might be you, a parent/guardian, or a spouse/domestic partner).
- Please be ready to upload a digital copy, picture, or screenshot of the front and back of your insurance card.
We ask for your health insurance information to expedite care in case we need to provide referrals off campus for specialist care, or for any labs that we send for processing off campus at Quest Diagnostic Labs. If insurance information is not provided, the student health center may be unable to complete recommended labs or provide specialist referrals.
Please note that starting August 3, 2026 there will be a $5.00 office visit fee at the Student Health Center. We also charge fees for on site laboratory tests, vaccinations, medications, and some medical procedures and consultations provided entirely within our clinic.
All on site fees will be charged directly to your LC student account, and you will have access to a statement that you can submit to your insurance company for reimbursement. We do not direct bill insurance for any on site costs.
If you have the school sponsored Student Health Insurance Plan through PacificSource, onsite fees will be reimbursed at 100% when you submit a claim. Claims submitted to other insurance plans will be considered out of network.
All actively enrolled undergraduate students are automatically enrolled in and charged for the Student Health Insurance Plan through PacificSource, with summer coverage included with the spring coverage. All undergraduate students attending classes in the Fall or Spring semesters must have the Student Health Insurance Plan or health insurance that is comparable to the Student Health Insurance Plan (please confirm directly with your insurance company whether it meets the criteria and covers providers and required services in the Portland metro area).
Students do not need to be enrolled in the Student Health Insurance Plan to receive care at Lewis & Clark Student Health or Student Counseling Centers. All students are welcome to submit a claim to insurance for Student Health fees charged to their student account, and these services will be covered at 100% if enrolled in the Student Health Insurance Plan through PacificSource. Claims submitted to other insurance plans will be considered out of network.
Starting August 3, 2026 the Student Health Center will charge a $5.00 office visit fee in addition to existing low fees for some on site services. The Student Counseling Center provides all services at no charge.
Lewis & Clark has partnered with PacificSource to offer a health insurance plan for students that contains robust benefits that support the health of college students. The plan provides excellent value as well as access to a wide network of providers both in Portland and across the nation. We encourage you to review the Student Health Insurance Plan and see if it meets your needs. Please note: Medicaid plans from states other than Oregon and Washington usually do not meet the comparability requirements.
If you do not want to be enrolled in and charged for the school-sponsored health insurance plan, you must complete the online waiver.
- For students starting coursework in the fall, the waiver form can be accessed from the Declining Insurance Coverage website beginning June 12, 2026, and before the waiver deadline on September 14, 2026.
- If you complete the waiver in June, the charges for the Student Health Insurance Plan will not appear on your student account statement.
Students have only one opportunity each academic year to waive enrollment in the school-sponsored health insurance plan. The waiver must be submitted by the waiver deadline each academic year (September 14, 2026 for the 2026-27 academic year). We encourage our undergraduate students to confer with their parents or guardians to evaluate whether or not they need the school-sponsored health insurance plan.

Please note: If you are a visiting student from Hoksuei Gakuen, Waseda, SACM, the Embassy of Kuwait, Rikkyo or Konkuk, you will have health insurance coverage through your sponsoring organization so you will not be enrolled in student health insurance. You can disregard the above paragraphs about insurance issues.
We want to encourage attention to your overall health and well-being, and Lewis & Clark provides multidimensional support to support your personal and academic success. We encourage you to become familiar with mental health consultation, crisis support, and psychotherapy available at the Student Counseling Center.
