New Student Information

Please provide a signed certification from a physician or nurse practitioner providing a medical reason for the exemption from the measles/MMR vaccine (history of severe allergic/anaphylaxis from past immunization, immune compromised state, etc.). Must include reason for medical exemption and denote permanent or temporary exemption (including date that temporary exemption will be lifted).

To submit your signed certification for a medical exemption:

• Upload to the Health Information Portal - select “immunization records”,
• Fax 503-768-7167, or
• Email to health@lclark.edu.

Oregon Health Authority requires a signed Vaccine Education Certificate to be on file at the Health Center. This can be accomplished in one of two ways:

• Have your practitioner sign a Vaccine Education Certificate. Available for download at Oregon Health Authority’s website.

OR

• Watch an online College Measles Module (scroll to bottom of page to locate). Print out and sign your Vaccine Education Certificate at the end of the module.

To submit your signed Vaccine Education Certificate:

• Email to health@lclark.edu.
