Documentation Guidelines:
Please do not delay meeting with us if you do not have documentation. Lack of documentation may delay the consideration of certain accommodations, but need not delay the initial meeting.
For academic accommodations, please provide documentation or a letter from your provider with your diagnosis/es, the impact of these on your ability to access your education, and recommendations for accommodations.
For residential accommodations, please ask your provider to complete the LC Residential Accommodation Documentation Form and return to you for submission through Accommodate
